Pro and the Health system pilot tier are in closed pilot. We onboard a small number of design partners while we refine the regulated-industry deployment posture. The free tier stays free, no matter who buys what.

Why the pricing is structured this way
Caregivers and individual patients run free, on-device. The whole point is people who currently cannot use a patient portal unaided. Gemma 4 nano + Ollama on a Mac, no API key, no metered cost, no data leaving the device. We don't charge the user we built Spectra for.
Pro is for advanced caregivers and individuals who want Gemini Live latency for daily use and don't want to run Ollama. Cloud Gemini Live is around 25 cents per minute of streamed audio; the Pro fee covers an audited 30 hours per month plus a margin so we can offer the free tier at zero variable cost.
The Health system pilot tier is how hospitals procure. HSE, Irish private healthcare, EU public hospitals, NHS Trusts, US patient-portal vendors, disability organisations and banks need bulk seats, DPA (or BAA for US deployments), and FHIR R4 outputs that drop into the clinical record system they already run. Local mode (Ollama + Gemma on the user's machine) is supported for data-residency-strict clinics that cannot send a single byte off the device.
